PHYSICAL THERAPY
Home Health Physical Therapists provide services and treatment in the patient’s home and surrounding environment. Unlike a rehabilitation setting or even an outpatient clinic, unique emphasis is placed on functional mobility in and around the patient’s residence where he/she will spend most of his/her time.
The community training incorporates the challenges of uneven surfaces and normal distractions, plus focusing on the patient’s own goals with regards to mobility.
Our Physical Therapists seeks to improve the patient’s mobility toward his/her prior level of function (PLOF).
Our agency adheres to the Medicare guidelines on skilled functional outcomes and documentation requirements.
A Physical Therapist will conduct a comprehensive evaluation assessment to determine the best way to improve the patient’s function and identify the patient’s or family’s own goals.
The patient (or family) is fully involved with developing the plan of care that the primary care physician will ultimately approve.
Our Therapists are highly skilled coming from a variety of clinical backgrounds, experienced on best techniques to motivate the patient to succeed.
Our Therapists will also make recommendations on equipment and train the patient on its proper use, and will train the family member or caregiver to help the patient maintain an exercise program.
